The residents and staff of SEM Haven Health & Residential Care Center welcome you to our website. We are proud of our home which includes Nursing, Assisted Living, Short-Term Rehabilitation and Alzheimer's/Memory Care.
We have made a commitment to the Eden Alternative™ philosophy of care, where residents are the center of daily life and care is provided in an atmosphere alive with pets, plants, family and wonderful friendships.
In 2001, our 12-member volunteer Board of Directors began planning a construction and renovation project that has resulted in a vastly improved state-of-the-art community.
Resident and family satisfaction surveys conducted by the Ohio Department of Aging placed SEM Haven near the top of all homes in Ohio resulting in commendations from the Ohio Health Care Association and Senator Tom Niehaus!

Ohio Department of Health Survey
Congratulations to SEM Haven staff on a PERFECT health survey in both our nursing and assisted living communities. The Ohio Department of Health conducted the survey October 26-30, 2009.
This demonstrates our commitment to excellence in resident care.
SEM Haven Achieves 5 Star Rating
Please go to the CMS website, www.medicare.gov/NHCompare (nursing home compare). Nursing homes overall quality rating is based on ratings in three areas: health inspections, quality measures and staffing. There are many variables that affect a facility’s score so the state long-term care ombudsman recommends that you visit the home, observe and ask questions. SEM Haven received an excellent rating of 5 Stars placing us in the top 10% of all homes in the country!
SEM HAVEN RECEIVES QUALITY AWARD!
SEM Haven is one of only 18 Ohio Health Care Association Members to have earned the American Health Care Association’s Step I Quality Award for 2008. It was presented during the AHCA Annual Convention in Nashville in October. The AHCA Quality Award is a distinction given to member organizations for applying principles of continuous quality improvement. The Quality Award program is directly aligned with the Quality First Initiative. Participation in the award program helps providers adhere to the principles of Quality First and put in place processes that generate positive outcomes. Applicants are evaluated based on criteria adopted from the Malcolm Baldrige National Quality Award (MBNQA) Health Care Criteria for Performance Excellence.
The quality award has been designed as a progressive three-step process to encourage the continuous learning and development of integrated quality systems to achieve performance excellence. Each progressive step requires a more detailed and comprehensive demonstration of quality integration and performance. Step I award recipients are able to describe their mission, characteristics, and key challenges and to recognize the relationship of these factors to their ability to achieve performance improvement.
